Adult Learning Disabilities Health Service

The Adult Learning Disability Team is based within Bolton Council’s Active Case Management teams, and works alongside social care colleagues. Together we aim to provide a service to adults with a learning disability across a wide range of health and social care needs.

Age	16 to 99 years
Cost	As we provide Health Support and are part of the NHS there is no charge for this service.
Referral details	Referrals can be made via telephone or in writing to the STARs Team, see contact details.
Special provisions	The service can only be accessed by adults with a learning disability. Support is offered following a specialist assessment and the duration of involvement is dependant on need.
